Clearing Up Any Confusion With National’s Latest Reminder Regarding Medical Forms
Dear Jamboree Parents, Scouts, and Venturers,
Last week you may have received a general reminder from the National Jamboree for medical forms, curriculum activity registration, and a photo to upload in your Summit account. For medical forms, please follow the directions previously communicated to you to send the complete medical form (Parts A, B, and C) and a copy of both sides of your health insurance card to the Waterloo Scout Office (original message below). This will ensure that we have medical form copies for the leaders while traveling to and from Jamboree, and touring in Washington D.C.
We regret any inconvenience or confusion the message from National jamboree may have caused, and thank you for your patience while everyone gets ready for Jamboree.

Original Message and How You Should Handle Your Medical Forms
Dear Jamboree 2013 Parents and Participants:
We understand that some of you have been having problems getting to the medical forms on your Jamboree web account. There appears to be technical issues that create the difficulties in accessing and printing the Jamboree Medical forms. Our sincere apologies for the inconvenience this has caused you. We certainly understand your situation and are working with National as well as individuals who call into the office. The new online system supports Internet Explorer Version 9 or later, Firefox version 14 or later and Chrome version 21 or later. You may need to upgrade browser if you are currently using an older version. If you encounter difficulties, or cannot find the form, please call Jean Draude at the Waterloo Scout Office at 319-234-2867 or toll free 877-754-4079, and we will help in any way we can to access the forms for you. Medical forms dated from July 24, 2012 to April 15, 2013 will be accepted.
The on-line health form instructions ask for parts A and B to be submitted on-line, with Part C to be filled out by your doctor and then sent to B.S.A. in Irving, Texas. If you do that, we will not have a copy for us to use while traveling and touring. Instead please do the following:
- After filling out Parts A and B, please print a copy of them before submitting on-line.
- Have your doctor fill out Part C. information from a physical examination any time after July 24, 2012 is acceptable for use.
- Copy both sides of your health insurance card
- Send Copies of Parts A, B, and C, and the health insurance card ( both sides) copy to the Council Office in Waterloo at the following address by March 1, 2013.
The council will make needed copies for leader access while traveling, and then forward the forms to B.S.A. in Irving Texas. If you have already submitted your forms on-line and sent Part C to B.S.A., please send us your copies of Parts A, B, and C, and the copy of your health insurance card (both sides of ins. Card) to the council office by March 1st. You can also email them to Jean at [email protected] or fax at 319-234-0153. We apologize for any problems this may, or already has caused you.
