Jamboree 2013 Medical Information

Dear Jamboree Parents,

We learned earlier this week that medical forms for 2013 Jamboree will not be available to download from your Scout’s or Venturers online Jamboree account until mid-December at the earliest. Anyone that can’t wait until then can use the current BSA medical form located at http://www.scouting.org/filestore/HealthSafety/pdf/whole.pdf.

Parents need to fill out Parts A and B, and then have your physician fill out Part C.

Please make sure (1.) that you have registered for Jamboree on https://summit.scouting.org/en/Jamboree2013/Pages/default.aspxline with the email address you used when registering. If you have changed emails or need to enter a new one, please update your profile at the above web site as well.
( 2.) The completed form should be delivered no later than March 1, 2013 to the following address:

Physical examinations must be conducted on or after August 1, 2012 for the medical forms submitted for 2013 Jamboree.

It is preferred that parents wait until the form is available online so the printed copy contains the participant’s registration information. This option is being offered for situations where waiting for the individualized forms to become available creates problems.
